TeleHealth with total transformation of the clinical model can help an organization achieve excellent patient outcomes, decrease hospitalization rates and improve the bottom line.

Teamwork and consistent, sustained involvement from senior management is the key.  Clinicians must first be confident that they are actually providing better care with technology. The concept of a visit must be expanded to include remote visits and telephone contacts.  Fewer home visits does not mean less care for the patient. Build a bond with patients and family members that is supported by remote ?intensive care? and with the knowledge that you will provide immediate follow up to any call.  Through these efforts you can build a trusting relationship with your patients that will prevent unnecessary hospitalizations and allow you to manage patient care in a timely manner with appropriate home care interventions.
